Vet Companion

Veterinary clinical notes and pet medical records explained

Turn concise clinical entries, abbreviations and repeated visits into a clearer timeline of what was recorded, what was done and what was planned.

We explain the record as written. We do not correct clinical judgement, infer missing events or decide whether the care was appropriate.

Written for clinical continuity

Medical records are often brief rather than reader-friendly

Clinical notes are created to record care and help veterinary teams follow a case. They may use shorthand, repeated abbreviations and short entries that make sense to professionals but feel fragmented to an owner.

  • Place visits and telephone notes into date order
  • Expand clear abbreviations without guessing
  • Distinguish observations, assessments, actions and plans

What we can explain

Information found across a clinical history

The exact layout varies by practice. We work from the entries and attachments included in the copy you provide.

Consultation entries

The date, reason for the visit, history given and examination findings recorded.

Assessments and possibilities

Confirmed, suspected, differential or rule-out wording used by the clinician.

Tests and results

Investigations ordered, results entered and later comments linked to them.

Treatment and medication

Procedures, medicines and recorded instructions across different visits.

Calls and messages

Telephone advice, updates, appointment notes and communication with the owner.

Plans and follow-up

Monitoring, rechecks, referrals and actions the record says should happen next.

A calm explanation can help you prepare questions without changing the professional advice in the original document.

Make a readable chronology

Follow what changed from visit to visit

We can arrange entries by date and label the role each piece of information plays. We preserve uncertainty words such as “possible”, “suspected” and “cannot rule out” instead of turning them into a definite diagnosis.

  • Date and authorWhen the entry was made and by whom, if shown
  • HistoryWhat the record says was reported
  • ExaminationFindings documented at the consultation
  • ActionTests, treatment or communication recorded
  • PlanMonitoring, review or referral written next

Questions to take back

Feel more prepared to ask

These are neutral prompts for the relevant veterinary practice, specialist, laboratory or insurer—not veterinary, legal or financial advice.

  • Which entry records the main diagnosis or concern?
  • Was this test completed, recommended or only discussed?
  • What does this abbreviation mean in this particular note?
  • Which medication instruction is the latest one?
  • Is an attachment, laboratory report or image report missing from this copy?

Clear boundaries

Explanation, not a judgement about care

We do not decide whether the record is clinically correct, complete, negligent or evidence of poor care. We cannot reconstruct events that were not written down or determine what a professional intended beyond the wording.

Frequently asked questions

Before you upload

Can you explain a full pet medical history?

Yes. We can create a structured timeline from the complete records you provide. Very long histories may need to be divided into clearly defined periods.

Can you decode every abbreviation?

We explain shorthand when its meaning is sufficiently clear from the context. We will not guess when an abbreviation could mean more than one thing.

Can you tell me whether a note is accurate?

No. We can show what the note says and flag an apparent inconsistency, but the practice must address factual corrections and a qualified professional must address clinical questions.

Can you compare records from two practices?

We can combine the documented timelines and show where wording or recorded information differs. We cannot decide which clinical opinion is correct.

Will you explain comments about a possible diagnosis?

We can explain the level of certainty used in the wording. We do not confirm or rule out the condition.

Should I remove personal information first?

You may remove information that is not needed for the explanation, such as your address, financial details or insurer policy number, while keeping the clinical context complete.
